Many families seeking to place a loved one in a nursing facility or an assisted living facility will find themselves in need of assistance. Placement of a relative can be an emotionally difficult and stressful task. Families often discover they lack the time or experience to properly place their loved one.
- The process of nursing home placement begins with the PRI/SCREEN, which are forms required by the New York State Department of Health. A qualified registered nurse assessor will evaluate the patient's nursing, medical and emotional needs.
- Finances are an important consideration. If indicated, assistance is available in obtaining Medicaid.
- When it has been determined what level of care the patient requires, the family may request assistance with placement.
