Assistance
Many families seeking to place a loved one in a nursing facility or an assisted living facility will find themselves in need of assistance. Placement of a relative can be an emotionally difficult and stressful task. Families often discover they lack the time or experience to properly place their loved one.
The process of nursing home placement begins with the PRI/SCREEN, which are forms required by the New York State Department of Health. A qualified registered nurse assessor will evaluate the patient's nursing, medical and emotional needs. |

Finances are an important consideration. If indicated, assistance is available in obtaining Medicaid. |

When it has been determined what level of care the patient requires, the family may request assistance with placement. A placement coordinator will consider the patient's and family's wants and needs, financial considerations, as well as geographic factors to develop a list of suitable facilities. |

Placement assistance is not complete until a satisfactory placement is arranged. |
|
Geriatric Care Managers are available for follow-up evaluations after placement, in order to assure the family that their loved one is receiving appropriate care. |
